Preparing For Discharge:
Throughout your hospitalization, we have attempted to provide you with information essential to your recovery and subsequent discharge.

Goals for your discharge plan will be developed with you, your physician, professional nursing staff and any appropriate disciplines necessary to achieve your transition from the hospital to your home.

If you have any questions, or problems related to your discharge, please let your nurse know.

Friends or relatives assisting you with your discharge are to pick you up in the patient discharge area located in the rear of the building, just beyond the loading dock.

Child Car Seat Law:
New York State law requires that all children under the age of four be safely secured in a federally approved child restraint while traveling in a private passenger vehicle. Please be properly prepared if you are bringing a newborn or child home from the hospital.
