FT opportunity for NYS licensed MSW to provide psycho-social evaluation, counseling and appropriate referral services to in-patient, out-patient and nursing home residents. Serves as a liaison with community based service agencies and treatment facilities. Must be able to work effectively in a multi-disciplinary treatment team.

MSW or CSW with previous health care experience in acute and/or long term care settings preferred.
